of practice, we are available to provide these services, however

arrived at in our intake and evaluation process.
We work in concert with all health care providers and request
diagnosis for health care conditions, as needed. We do not
recommend discontinuing any prescribed course of treatment in
clients who decide to work with us but instead refer individuals
to the prescribing practitioner involved for these type of decisions.
If a client client desires a diagnosis from a licensed physician,
chiropractor, or acupuncture practitioner, or services from a physician,
chiropractor, nurse, osteopath, physical therapist, occupational therapist,
dietician, nutritionist, acupuncture practitioner, athletic trainer,
or any other type of health care provider, the client may seek such services at any time.
We are also available to provide information about professional
alternative health practices/practitioners that may support your
personal wellness/health goals. You have a right to change
practitioners after services have begun at any time, although
the risks and possible health consequences of these changes will
be explained to you. Your health insurance may also have some
limitations or guidelines regarding such changes.
If at any time, for any reason, there is a change in the provider
of your services, you have the right to have assistance with the
coordination of your transfer to another practitioner.
Further rights and responsibilities of the client as well as the
Winter Health and Wellness or Afton Alternative Associate practitioner
offering the service will be explained at the time of intake.
With the following considerations, rights and responsibilities
in mind, we have found that our clients feel well informed,
empowered to make decisions and secure and comforted about going
forward with the services of their choice. We see this process
as critical to our client's taking control of their health.
Being a chooser, deciding and making commitments to improve
your health is empowering. Good health is based on this core dynamic.
